The City School District of Albany is dedicated to providing a quality
education to all students. As part of our commitment, and in compliance
with the federal No Child Left Behind Act, we would like to inform you
about extra school support called Supplemental Education Services (SES).

Your child is eligible for SES if he or she attends
a Title I School in Need of Improvement in English language arts and/or
math, and is eligible for free or reduced-price meals. The eligible
schools for 2011-2012 are Albany School of Humanities (ASH), Delaware
Community School, Giffen Memorial Elementary School, William S. Hackett
Middle School, Stephen and Harriet Myers Middle School, North Albany
Academy, Philip Schuyler Achievement Academy and Thomas O’Brien Academy
of Science and Technology (TOAST).
These services are free for your child. We expect to provide
help/tutoring to all eligible students who ask for it. If there are more
students than funding, students with the greatest academic need will
have first priority.
If your child is having difficulty in ELA, reading and/or math,
then SES will provide the tutoring that your child needs to be
successful. Click

programs that are approved by the New York State Education Department.
Each program is different.
You will need to review the information and do some research to
find the program that is the best one for your child. To help you
decide, think about the answers to the following questions:

When and where does the tutoring take place?

How far away is the program from our home,
since no transportation
is provided by the school?

How often will the tutor meet with my child and
how long will each session be?

What programs, by grade level and subject area,
are provided by the tutor?
-
What type of instruction is used (individual,
small group, computer-based)?
-
Does this program meet my child’s needs?
In order to enroll your child in this program, please complete and
return the
Supplemental Education
Services Form to the school in care of the principal.
When completing the
Supplemental Education Services Form,
the following steps should
be taken:

Choose a first-, second- and third-choice
provider from the
current state-approved list.

Contact the provider of your first choice and
make an appointment to learn about the program.
This can be an on-site visit or a phone inquiry.

If you are not pleased with that program,
contact your second and third choices.

Print your name, address and phone number, and
provide your signature and date on the form.
Be sure to fill out all information – student name, school
and grade. The office will
provide a student ID number.

Return the
Supplemental Education
Services Form
to your child’s school in care of the
principal.
The school will then forward the
Supplemental Education Services
Form to the district office.
The district office will notify your chosen provider when your
child’s eligibility is verified and the application is processed.
The provider will then notify you with details, including when
SES tutoring services will begin.
This will happen shortly after you submit the
Supplemental Education Services
Form
to the school.
