Albany High School Registration
Required forms and documents

In order to register your child for school, you will need to complete the following forms and bring proper identification with you to the Central Registration, located at 75 Watervliet Ave., off of Central Avenue. Students will not be able to attend school until all the necessary forms are completed and immunization records are on file at Central Registration.Please be sure to read the registration form instructions carefully as there are grade- and situation-specific application forms that do not apply to everyone. Note: To download a form, click on the name of the form listed below. You will need Acrobat Reader to view the forms.

 

Student registration form: Please complete and sign.

Request for Records form: Please complete and sign.

Emergency Management Form: Please complete and sign.

Health History form: Please answer all questions and sign.

Photo form: Please complete and sign.

Student Physical form: Your child's physician/family doctor must complete this form. A student may not participate in gym classes until the physical form is completed.

Medication Permission form: Your child's physician/family doctor must complete this form.
