Required identification for student
registration
To register your child for school in the
City School District of Albany, you will need to bring proper
identification with you to Central Registration.
New York
State law requires that the following identification documents be
presented prior to registering child for school:
1. Proof
of birth (no copies, please).
2.
Parent/Guardian photo identification is required.
3. Proof
of address (2 items) which must include the name and address of the
parent or guardian and must be dated 30 days prior to registration.

Telephone bill or the "Welcome Letter" from the telephone company

Utility
bill (electric, gas)

Cable
television installation receipt
-
Furniture rental agreement (i.e., Aaron's, Rent-A-Center)

Auto
insurance policy
-
Original
lease agreement (acceptable only from an apartment complex or real
estate company)

NYS
Driver's license issued within the last 30 days or change of address
sticker on the reverse side

Payroll
check (dated within the last two weeks)
We are
sorry, but we cannot accept the following as proof of identification:

Official immunization records.
5.
Custody papers, if applicable. (Court order of custody is acceptable.)

If the
student is not your biological child, documentation must be
presented which proves a permanent and total transfer of custody and
control have been achieved.

Notarized letters or petitions for custody are unacceptable.
6. Report
Card or transcript for all new students entering middle school.
