applicants for admission or professional agreements with the City
School District of Albany are hereby notified that this district

If you believe that you or your
child have been subject to discrimination on the basis of a disability

the Albany City School District, you have the right to file a complaint
with the district's 504 and ADA (Americans with Disabilities Act)
Compliance Officer.
Upon receipt of a written
complaint, the Compliance Officer will conduct an independent
investigation into the facts alleged, issue a report, including findings
of fact and, where applicable, oversee the implementation of a timely
