What’s Next? Transition Planning for Life After High School

We are pleased to announce a virtual community conversation in partnership with Wildwood Programs’ School Age Family and Community Engagement (FACE) Center.

This virtual session is open to students with disabilities in grades 8-12, their families and community partners. The session will give pointers on working with your school district’s Committee on Special Education to:

• Assess your student’s strengths, interests and needs;
• Establish post-high school goals and yearly goals for your student; and
• Identify appropriate transition services and coordinated activities for your student
