Important forms for basketball, competitive cheer

The official start of high-risk winter sports is still on hold, in accordance with Albany County Health Department guidelines. However, the City School District of Albany would like to share two forms that parents and guardians will need to submit if the county approves the start of basketball and competitive cheerleading.

Parents and guardians will need to complete the first form to provide their consent. A healthcare provider must complete the second form. Both are required before a student-athlete can begin practicing.

The county has indicated that high schools can begin tryouts and practices for high-risk sports once the following COVID-19 criteria has been met: 

  • The county’s seven-day rolling average COVID-19 positivity rate is at or below 4%, and 
  • The region’s hospital capacity is above 15%

Also in accordance with the county’s guidance, the district’s Board of Education must approve participation in each high-risk sport. The board is expected to consider a resolution at its Feb. 11 meeting.
